He was shot in the head by a crazed fan who heard Phil say that someone needs to shoot Dime bag while he was high and figured he should be the one who should do it. After he shot him a security guard shot him and he was killed as well.

With regard to the "clicky" kicks, I'm pretty sure they use something (cant remember the name) which is basically a kevlar disk you put on the skin of the drum where the pedal hits it, giving a more consentrated sound.
But hey I'm no expert.
