I'm a dentist too and I've never used a Hammer to extract someone's teeth. For that case you make an incision or osteotomy for that matter. xD

what the hell is that!!! u must change ur dentist,i'm dentist and never saw someone applying that much pressure to extract a teeth,mandible may break and it happens a lot for such pressure,he even used a hammer!!! haha oh my god!
